YAA Banquet Brochure AD Form

If you would like to highlight your business or simply congratulate your awardee on his or her accomplishment, you may help support our program by submitting an AD to the Brochure that will be distributed at the Banquet.  Download the FORM HERE and send it in.  You can do it all digitally on EMAIL and PayPal OR print the form and send it in the mail with your AD image.
